Command Level Preparation Program Application

Participants in the LDIG Command-Level Preparation Program should meet the following minimum qualifications:
1. Be in a sworn position or the civilian equivalent at a public or private criminal justice-related agency or company,
2. Hold a managerial position at the rank equivalent of Lieutenant or higher in that agency or company,
3. Have at least three years’ experience in that rank,
4. Have a minimum of five years of experience in a bona fide criminal justice agency,
5. Be recommended in writing by the agency head or an authorized designate, and
6. Have completed and submitted an LDIG Application and Recommendation.
Instructions:
1. Complete the following information form
2. Print the Recommendation Form and have your Agency/Department Head or an Authorized Designee sign it
3. Scan the signed Recommendation and include it in the "Upload Signed Recommendation Form Here" block below
4. Click "Submit" at the bottom of this page
